Appointment Policy

We operate by appointment. Please call (608) 348-9581 to schedule your pet today! 

Safety Policy

For the safety of our staff, clients, and all pets in our care, we require all pets to be current on vaccinations and arrive to the clinic on a leash or in a carrier. 

Payment Policy

We require full payment at the time that services are rendered.  We accept Visa, MasterCard, Discover, Care Credit, cash, and personal checks.  

Prescription Policy

Family Pet Hospital will provide a written prescription for veterinarian prescribed medications at your request.  We cannot fax or call prescriptions to outside pharmacies. 

Rabies Vaccination Policy

In order for our staff to safely handle your pet, Family Pet Hospital requires they be current on their rabies vaccination.  If you cannot provide proof of a current rabies vaccine, your pet will receive one at the time of your appointment.  

Cancellation Policy

Missed appointments and late cancellations often mean other patients in need have had to wait. If this becomes a common occurrence pre-payment will be required.  

Privacy Policy

Family Pet Hospital recognizes the importance of protecting the personal privacy of its clients, patients, and individuals who visit our site. Neither Family Pet Hospital nor our web hosting company collect or distribute any information to third party vendors or internet businesses.
