Family Pet Hospital Policies

We operate by appointment. 

Please call (608) 348-9581 to schedule your pet today! 

For the safety of our staff, clients, and all pets in our care, we require all pets to be current on vaccinations and arrive to the clinic on a leash or in a carrier. 

We require full payment at the time that services are rendered.  We accept Visa, MasterCard, Discover, American Express, Care Credit, cash, and personal checks.  

Family Pet Hospital will provide a written prescription for veterinarian prescribed medications at your request.  We cannot fax or call prescriptions to outside pharmacies. 

In order for our staff to safely handle your pet, Family Pet Hospital requires they be current on their rabies vaccination.  If you cannot provide proof of a current rabies vaccine, your pet will receive one at the time of your appointment.  

Missed appointments and late cancellations often mean other patients in need have had to wait. If this becomes a common occurrence, pre-payment will be required.
